A 55-year-old female presented with long-standing poor vision in the left eye. She
had a history of a previous retinal detachment (RD) repair in the left eye. On examination,
there was evidence of a chronic macula-off RD with macrocysts (A). Layering of heme was noted within the cystic structures (A, B). Fluorescein angiography was performed and revealed abnormal leaking vessels within
the macrocysts (C). Macrocysts are uncommon findings in chronic RDs and are thought to be secondary
to degeneration and splitting of the retinal layers. Because they are not true cysts,
they do not need to be excised or drained during RD repair.
